Body Measurement Form

Note: Please indicate the unit of measurement used, whether inch or centimeter.

1. Full Shoulder Measure back at end of shoulders.	2. Chest Measure around body well up under arm holes.	3. Waist Measure around waist line.	4. Sleeves Measure sleeves from shoulder seem to length desired.
5. top Length Measure from collar seam to length desired.	6. Hips Measure around Hips at widest point of seat but not tight.	7. Length Measure from top of waist band to bottom of cuff. (including boy, gilr)	8. Thigh Measure width around the thigh as shown.
9. Neck Measure around neck and add ½ inch.
